ISHM Fees
To pay application and renewal fees listed below, send your payment to ISHM via email, fax (928-726-0420), or phone (877-201-4053). All amounts are U.S. denomination.

The ISHM billing year runs October 1 through September 30 of the following year. If your acceptance date is October 1 through February 28 you will be assessed one-half the yearly renewal fee. If you are accepted March 1 through September 30 you will not be billed for the current year but will be billed for the following year's renewal.
